Request a sickness certificate (fit note)

You must give your employer a doctor’s ‘fit note’ (sometimes called a ‘sick note’) if you’ve been ill for more than 7 days in a row and have taken sick leave. This includes non-working days, such as weekends and bank holidays. If we are unaware of the illness, then we may ask you to book an appointment with a GP first.

If your employer insists on a certificate from your doctor for illness between 1 to 7 days, this service is not covered by the NHS and a charge will be made which currently is £30.
